At Partners in Family Medicine in Shelby Charter Township we are dedicated to improving and maintaining your and your family's health through preventative care and treatment of chronic diseases. We treat patients ranging from newborns to geriatrics. In-office services include on-site digital X-Ray, laboratory services such as cholesterol and blood sugar screening, immunizations and wellness visits. Sick appointments can usually be made the same day and we also offer convenient evening hours. Our office has been designated as a Patient Centered Medical Home.
Dr. Koby is a board certified family physician. Dr. Koby graduated from the Michigan State College of Osteopathic Medicine and completed his residency at Mt. Clemens Regional Hospital. He is on staff at McLaren Macomb.
Dr. Wixon is a board certified family physician with a special interest in nutrition, pediatrics , women's health and preventative medicine. Dr. Wixon graduated from the Michigan State College of Osteopathic Medicine and completed her residency training at St. John Macomb Hospital. She is on staff at McLaren Macomb.
Juli Grover, nurse practitioner is the newest member of our team. She obtained a Doctorate in Nursing Practice from Oakland University where she also completed her undergraduate and Master’s degree in Nursing. She comes to us with over 8 years of experience as a nurse practitioner and previously as an Emergency Room Nurse for 6 years. She loves taking care of people of all ages and helping them to potentiate their health. She is married with two children and views her patients like family.
Ms. Prusakiewicz (pronounced proos-a-kev-itch) obtained her boad certification from the American Academy of Nurse Practitioners after passing the Family Nurse Practitioner Board Exam in 2013. She completed her Master of Science in Nursing at Oakland University and her Bachelor of Science in Nursing at the University of Michigan. She worked previously at another family practice office and urgent prior to joining PFM. She also worked as a RN in the Pediatric Intensive Care Unit at Children's Hospital.
Nurse practitioners obtain an undergraduate degree in nursing and typically gain working experience as a Registered Nurse before attaining a Masters of Science in Nursing while some will continue on to acquire a Doctor of Nursing Practice.
Where physicians and nurse practitioners differ most is that doctors will complete four years of medical school and then complete at least three years of residency gaining clinical experience and furthering their education.
Our physicians and nurse practitioners are all board-certified and work closely together to provide our patients with the highest level of care. Our providers strive to work with their patients to achieve better health outcomes by providing a comprehensive approach to your health.
Quality care for our patients is our top priority. The office will confirm all appointments the day before. We understand emergencies happen but respectfully request you call to cancel your appointment so your slot can be given to the next patient. If you are more than 20 minutes late to your scheduled time you will be asked to reschedule. All no show appointments are subject to no show fees.

After Hours Care
If you need to be seen after normal office hours proceed to Stony Creek Urgent Care located at 8180 26 Mile Rd, Suite 100 in Shelby Twp at the corner of 26 Mile Road and Van Dyke. They can be reached at 586-710-3000.
Refill requests may be made during normal office hours by calling the office at 586-677-3310. Requests may also be sent from your pharmacy. Refills will not be given after hours or on weekends; patients should not use the after hours pager for requests.
Prescriptions may not be picked up by anyone other than the patient unless a Release of Information form is on file in your chart. Your representative must present a picture ID to pick up your prescription.
All copays and deductibles are due at the time of service. For your convenience we accept cash, check, Visa and MasterCard.
We participate with most major insurance companies. It is your responsibility to make sure that you have selected our office as your primary care provider prior to your appointment if necessary, otherwise your appointment may be subject to cancellation.
Your insurance policy is a contract between you and the insurance company. It is your responsibility to notify our staff if your bloodwork needs to be sent to a specific location other than McLaren Macomb. If you require a prior authorization for an outside test, we will do our best to work with you and your insurance company so that you receive full benefits but we cannot guarantee coverage by your insurance company on a test ordered by one of our physicians.
