Our mission is to provide you with personalized, high-quality care. At Partners in Family Medicine in Shelby Charter Township we are dedicated to improving and maintaining your and your family's health through preventative care and treatment of chronic diseases. We treat patients ranging from newborns to geriatrics. In-office services include on-site digital X-Ray, laboratory services such as cholesterol and blood sugar screening, immunizations and wellness visits. Sick appointments can usually be made the same day and we also offer convenient evening hours. Our office has been designated as a Patient Centered Medical Home.
Quality care for our patients is our top priority. We respectfully ask that you cancel your appointment within no less than 24 hours if you are unable to make it so that we may put other patients in that slot. As a courtesy we make every effort to confirm and remind you of your appointment via a text message or automated phone call. It is your responsbility to know when your scheduled appointment is. If you are more than 15 minutes late to your scheduled time you will be asked to reschedule. All no show appointments, including those that are not cancelled within 24 hours, are subject to no show fees.

Please call your pharmacy for refill requests or questions on your refill status. Refill requests may also be made via the patient portal. Refills will not be given after hours or on weekends; patients cannot use the after hours service for requests.
Prescriptions may not be picked up by anyone other than the patient unless a Release of Information form is on file in your chart.
All copays, deductibles and balances are due at the time of service. For your convenience we accept cash, check, Visa and MasterCard.
We participate with most major insurance companies. It is your responsibility to make sure that you have selected our office as your primary care provider prior to your appointment if necessary, otherwise your appointment may be subject to cancellation.
Your insurance policy is a contract between you and the insurance company. We participate with Quest Diagnostics. If your insurance requires your bloodwork to be sent to a different lab, it is your responsiblity to notify us before your blood is drawn. If you require a prior authorization for an outside test, we will do our best to work with you and your insurance company so that you receive full benefits but we cannot guarantee coverage by your insurance company on a test ordered by one of our physicians.
Referral requests should be made no less than 72 hours prior to your scheduled appointment. For diagnostic imaging, there are some insurance companies that require up to 10 days for their submission process. It is your responsibility to know what your insurance requires. When submitting requests to us we need the name, NPI, date of your appointment and reason for visit.
For Blue Care Network patients, you must have had at least one visit to our office in the calendar year and have seen one of our providers for the reason you want the referral before we can submit a Blue Care Network referral for you.
