Our mission is to provide you with personalized, high-quality care. At Partners in Family Medicine in Shelby Charter Township we are dedicated to improving and maintaining your and your family's health through preventative care and treatment of chronic diseases. We treat patients ranging from newborns to geriatrics. In-office services include on-site digital X-Ray, laboratory services such as cholesterol and blood sugar screening, immunizations and wellness visits. Sick appointments can usually be made the same day and we also offer convenient evening hours. Our office has been designated as a Patient Centered Medical Home.
Quality care for our patients is our top priority. You will receive a text message or automated phone call to confirm your appointment 72 hours prior and a text reminder or phone call the day before. We respectfully ask that you cancel your appointment if you are unable to make it so that we may put other patients in that slot. If you are more than 20 minutes late to your scheduled time you will be asked to reschedule. All no show appointments are subject to no show fees.

Refill requests may be made via the patient portal or by calling the office duing normal office hours. Requests may also be sent from your pharmacy. Refills will not be given after hours or on weekends; patients should not use the after hours pager for requests.
Prescriptions may not be picked up by anyone other than the patient unless a Release of Information form is on file in your chart. Your representative must present a picture ID to pick up your prescription.
All copays, deductibles and balances are due at the time of service. For your convenience we accept cash, check, Visa and MasterCard.
We participate with most major insurance companies. It is your responsibility to make sure that you have selected our office as your primary care provider prior to your appointment if necessary, otherwise your appointment may be subject to cancellation.
Your insurance policy is a contract between you and the insurance company. It is your responsibility to notify our staff if your bloodwork needs to be sent to a specific location other than McLaren Macomb. If you require a prior authorization for an outside test, we will do our best to work with you and your insurance company so that you receive full benefits but we cannot guarantee coverage by your insurance company on a test ordered by one of our physicians.
